How a program admin can access the longitudinal continuity clinic forms for an individual trainee in the dashboard data export:

1. Download assessment data via “Export data for everyone, one person per file (zipped) or “Export data for trainees, one trainee per file (zipped)”
2. Within an individual trainee file:
   1. Filter column N (activity_title) to “LONGITUDINAL CONTINUITY CLINIC ASSESSMENT”
      1. This will show all longitudinal continuity clinic assessments
      2. The data extract is in long form, so each single assessment is represented in multiple rows, with each row representing a single question/answer of the form.
   2. Navigate to column J (assessment_event_id). If there is only one id, all of the rows shown pertain to a single assessment (see Screenshot 1). If there are multiple assessment_event_ids shown (see Screenshot 2), filter the column and choose one at a time.
   3. Navigate to column BA (question_prompt), column BE (answer_option_anchor) and column BF (value) to view the relevant questions/responses for the selected assessment
   4. Columns AA (rater_first_name) through AD (rater_job_title_during_event) provide information related to the rater who completed the assessment.
   5. Column R (created_by) indicates who created the assessment.
3. The above process will work for any specific activity or sets of activities you want to view data for.
